Description
- Treatment of type 2 diabetes mellitus
- Management of type 2 diabetes
- Lowers basal and postprandial plasma glucose
- Improves insulin sensitivity
- Reduces hepatic glucose production
- Decreases intestinal absorption of glucose
Dosage and Administration
- Dosage must be individualized based on effectiveness and tolerance
- Available in immediate release and extended release tablet forms
- Dosage varies for adults and children
- Maximum daily doses should not be exceeded
- Not to be used in patients with eGFR below 30 mL/min/1.73 m2
Interaction
- Potential interaction with Furosemide and Nifedipine
- Competes with cationic drugs for renal tubular transport systems
- May impact the pharmacokinetics of certain drugs
- Some drugs may lead to loss of glycemic control
Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure (GFR <30 mL/min)
- Acute conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders like nausea, vomiting, diarrhea, abdominal pain
- Liver function test abnormalities
- Skin reactions like erythema, pruritus, urticaria
Pregnancy and Lactation
- Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
- Metformin excreted into human breast milk
- Breastfeeding not recommended during metformin treatment
Precautions and Warnings
- Substantial excretion by the kidney
- Increased risk of lactic acidosis with renal impairment
- May lower vitamin B12 level
- Increased risk of hypoglycemia when used in combination with insulin
- Regular monitoring of renal function advised
- Careful use in elderly and pediatric populations
Use in Special Populations
- Dosage adjustment based on renal function in elderly subjects
- Confirmation of type 2 diabetes mellitus before treatment initiation in pediatric population

Overdose Effects
- Hypoglycemia not seen with doses up to 85 gm
- Lactic acidosis a medical emergency and must be treated in hospital
- Hemodialysis may be used for removal of lactate and M-min
