Indications
- Treatment of type 2 diabetes mellitus

Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Different mechanism of action from sulfonylureas
- Does not produce hypoglycemia
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose of 500mg or 850mg
- Dosage increases in increments of 500mg or 850mg

Pregnancy & Lactation

- Limited data on adverse effects on breastfed newborns/infants
- Caution and recommendations about breastfeeding during metformin treatment
Precautions & Warnings
- Excretion by the kidney and risk of accumulation and lactic acidosis
- Lowering of vitamin B12 levels
- Increased risk of hypoglycemia when used in combination with insulin
- Recommendation for regular monitoring of renal function
- Advice to stop therapy before surgery and reinstate after renal function recovery
- Not advised in conditions causing dehydration or serious infections or trauma
- Annual estimation of Vitamin B-12 levels for patients on continuous therapy
- Possible interaction with anticoagulants
Use in Special Populations
- Need for adjusted dosage in elderly subjects based on renal function
- No effect on growth and puberty detected during controlled clinical studies
- Recommended follow-up of metformin effect on growth and puberty in children
