Indications
- Treatment of type 2 diabetes mellitus in adults
- May be used in children from 10 years and adolescents
Mode of Action
- Biguanide oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage and Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
- Dosage increases in increments of 500 mg weekly or 850 mg every 2 weeks
- Maximum dosage for adults: 2000 mg per day
- Renal impaired patient precautions
Interaction
- Possible interaction with Furosemide and Nifedipine
- Potential interaction with cationic drugs and certain other medications
Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Acute conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Taste disturbance

Pregnancy and Lactation
- Not recommended during pregnancy

- Risk of NVmet accumulation and lactic acidosis with impaired renal function
- Lowering of vitamin B12 level
- Increased risk of hypoglycemia in combination therapy
- Regular monitoring of renal function

Overdose Effects
- Hypoglycemia not seen at normal doses
- Lactic acidosis is a medical emergency
