Description

Oramet is a medication used in the management of type 2 diabetes mellitus. It is available in immediate release and extended release tablet forms. It reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity.

Administration

  • The usual starting dose for adults is 500mg twice a day or 850mg once a day, given with meals.
  • The usual starting dose for children is 500mg twice a day, given with meals.

Renal impaired patient usage

Do not use in patients with eGFR below 30 mL/min/1.73 m^2

Interaction

  • Enhanced absorption with Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • No effect on Cimetidine pharmacokinetics

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure (GFR < 30 mL/min)
  • Acute conditions altering renal function

  • Hemolytic anemia
  • Lactic acidosis
  • Taste disturbance
  • Gastrointestinal disorders such as nausea, vomiting, diarrhea
  • Liver function test abnormalities

Pregnancy

Not recommended during pregnancy

Precautions warnings

  • Excreted by the kidney, regular monitoring of renal function is advised
  • Increases risk of hypoglycemia when used with insulin
  • Lower vitamin B12 levels
  • Not advised in conditions causing dehydration or in patients suffering from serious infections or trauma

Special population usage

  • Elderly - requires dosage adjustment based on renal function
  • Pediatric - use confirmed diagnosis before treatment, caution in children aged 10-12 years

Renal function monitoring

Creatinine clearance should be determined before initiating treatment and regularly thereafter

Overdose effects

  • Hypoglycemia not seen at doses up to 85g
  • Lactic acidosis, a medical emergency, may occur
