Indications
- Treatment of type 2 diabetes mellitus
- Management of type 2 diabetes in overweight patients
- Monotherapy for type 2 diabetes in adults
- Combination therapy for type 2 diabetes in adults
- Use in children from 10 years of age and adolescents
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Increases peripheral glucose uptake
Dosage and Administration
- Dosage individualized based on effectiveness and tolerance
- Starting dose for adults: 500 mg twice a day or 850 mg once a day
- Maximum daily dose for adults: 2000 mg
- Starting dose for children: 500 mg twice a day
- Swallow extended release tablet whole
Interaction
- No chronic co-administration information with Furosemide
- Enhancement of Nifedipine absorption
- Potential interaction with cationic drugs
- No effect on Cimetidine pharmacokinetics
- Certain drugs may produce hyperglycemia
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in vitamin B12 deficiency
- Taste disturbance
- Gastrointestinal disorders
- Liver function test abnormalities

Pregnancy and Lactation
- Increased risk of congenital abnormalities and perinatal mortality
- Metformin excreted into human breast milk
- No adverse effects observed in breastfed newborns/infants
- Breastfeeding not recommended during metformin treatment
Precautions and Warnings
- Substantial excretion by the kidney
- Risk of lactic acidosis
- Lower vitamin B12 level
- Increased risk of hypoglycemia
- Regular monitoring of renal function advised
- Hypoglycemia risk with combined therapy
- Reduced renal clearance during cimetidine therapy
- Potential interaction with anticoagulants
Use in Special Populations
- Adjustment of metformin dosage in elderly subjects
- Regular assessment of renal function in elderly subjects
- No effect on growth and puberty detected
- Caution recommended in children aged 10-12 years

Overdose Effects
- Hypoglycemia not seen up to 85g dose
- Lactic acidosis occurred in such circumstances
- Lactic acidosis is a medical emergency
