Indications
- Treatment of type 2 diabetes mellitus in overweight patients
- Monotherapy for adults with diabetes
- Combination therapy for adult and pediatric patients
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Improves insulin sensitivity
Dosage & Administration
- Dosage must be individualized based on effectiveness and tolerance
- Usual starting dose of 500 mg twice a day for adults
- Maximum daily doses of 2000 mg for adults
- Swallow extended release tablet whole
Interaction
- Enhances absorption of Pimin by Nifedipine
- Potential interaction with cationic drugs
- Certain drugs may lead to loss of glycemic control
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure
- Conditions altering renal function, tissue hypoxia, or hepatic insufficiency
Side Effects
- Not known: Hemolytic anemia
- Very rare: Lactic acidosis, decrease of vitamin B12 absorption, peripheral neuropathy
- Common: Taste disturbance, gastrointestinal disorders
- Very rare: Liver function test abnormalities, skin reactions
Pregnancy & Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities
- Metformin excreted into breast milk
- Breastfeeding not recommended during metformin treatment
Precautions & Warnings
- Substantially excreted by the kidney
- Lowers vitamin B12 level
- Increases risk of hypoglycemia when used with insulin
- Requires regular monitoring of renal function

Overdose Effects
- Lactic acidosis is a medical emergency
