Indications

  • Treatment of type 2 diabetes mellitus in overweight patients
  • Use in adults and children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight adult patients treated with metformin as first-line therapy after diet failure

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Improves insulin sensitivity
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerance
  • Usual starting dose of 500 mg twice a day or 850 mg once a day for adults
  • Usual starting dose of 500 mg twice a day for children
  • Swallow extended-release tablet whole without crushing, cutting, or chewing

Interaction

  • Enhances absorption of Pimin by Nifedipine
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure (GFR <30 mL/min)
  • Conditions with potential to alter renal function

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Taste disturbance
  • Gastrointestinal disorders
  • Liver function test abnormalities or hepatitis
  • Skin reactions

Pregnancy & Lactation

  • Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantially excreted by the kidney
  • Lowers vitamin B12 level
  • Increases risk of hypoglycemia when used in combination with insulin or insulin secretagogue
  • Regular monitoring of renal function advised

Use in Special Populations

  • Dosage adjustment based on renal function for elderly subjects
  • No effect on growth and puberty detected during controlled clinical studies
  • Caution recommended when prescribing to children aged between 10 and 12 years

Overdose Effects

  • Hypoglycemia not seen with Pimin doses up to 85 gm
  • Lactic acidosis is a medical emergency
