Indications

  • Treatment of type 2 diabetes mellitus in adults
  • Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
  • Reduction of diabetic complications in overweight type 2 diabetic adult patients

Pharmacology

  • Biguanide type oral antihyperglycemic drug
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage & Administration

  • Dosage must be individualized based on effectiveness and tolerance
  • Usual starting dose for adults is 500 mg twice a day or 850 mg once a day
  • Usual starting dose for children is 500 mg twice a day
  • Metformin XR should be swallowed whole
  • Dose adjustments for renal impaired patients

Interaction

  • No chronic co-administration information with Furosemide
  • Nifedipine enhances absorption of Sucomet
  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • Certain drugs may produce hyperglycemia and impact glycemic control

Contraindications

  • Hypersensitivity to active substance or excipients
  • Acute metabolic acidosis
  • Severe renal failure
  • Acute conditions altering renal function
  • Acute or chronic diseases causing tissue hypoxia

Side Effects

  • Hemolytic anemia
  • Lactic acidosis
  • Decrease of vitamin B12 absorption
  • Taste disturbance
  • Gastrointestinal disorders
  • Liver function test abnormalities
  • Skin reactions

Pregnancy & Lactation

  • Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities
  • Metformin excreted into human breast milk
  • Breastfeeding not recommended during metformin treatment

Precautions & Warnings

  • Substantial excretion by the kidney
  • Lowering of vitamin B12 level
  • Increased risk of hypoglycemia when used in combination with insulin
  • Regular monitoring of renal function advised in all diabetics
  • Caution in conditions causing dehydration or serious infections

Use in Special Populations

  • Adjusted dosage in elderly subjects based on renal function
  • Diagnosis confirmation before treatment in pediatric population
  • No effect on growth and puberty detected during clinical studies
  • Particular caution recommended for children aged between 10 and 12 years

Overdose Effects

  • Hypoglycemia not seen with Sucomet doses up to 85 gm
  • Lactic acidosis occurred in certain circumstances
  • Hemodialysis is the most effective method to remove lactate and Sucomet
