Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients treated with metformin as first-line therapy after diet failure
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity by increasing peripheral glucose uptake and utilization
Dosage & Administration
- Individualized dosage based on effectiveness and tolerance
- Usual starting dose of 500 mg twice a day for adults
- Usual starting dose of 500 mg twice a day for children
- Maximum daily doses of 2000 mg for adults
- Maximum daily doses of 2000 mg for children
- Dosage increases in increments of 500 mg or 850 mg
- Metformin extended release tablet dosage

Interaction
- No chronic co-administration information with Furosemide
- Potential interactions with various cationic drugs and renal tubular transport systems

Contraindications
- Hypersensitivity to the active substance or excipients
- Acute metabolic acidosis
- Severe renal failure (GFR < 30 mL/min)
- Acute conditions altering renal function
- Acute or chronic diseases causing tissue hypoxia
- Conditions causing dehydration or affecting renal function
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption and serum levels
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders like nausea, vomiting, and diarrhea
- Liver function test abnormalities or hepatitis
- Skin reactions like erythema and urticaria
Pregnancy & Lactation
- Increased risk of congenital abnormalities and perinatal mortality with uncontrolled diabetes during pregnancy
- Excretion into human breast milk
- Recommendation against breastfeeding during metformin treatment
Precautions & Warnings
- Substantial excretion by the kidney
- Lowering of vitamin B12 level
- Increased risk of hypoglycemia when used with insulin or insulin secretagogue
- Regular monitoring of renal function
- Avoidance in conditions causing dehydration, serious infections, or trauma
- Annual estimation of Vitamin B-12 levels
- Caution with sulphonylurea combined therapy
- Renal clearance reduction during cimetidine therapy
- Possible interaction with anticoagulants
Use in Special Populations
- Adjusted dosage based on renal function in elderly subjects
- Confirmation of type 2 diabetes mellitus before treatment with metformin in pediatric population
- No effect on growth and puberty detected during controlled clinical studies
- Caution recommended when prescribing to children aged between 10 and 12 years
- Regular assessment of renal function and creatinine clearance determination
Overdose Effects
- No hypoglycemia seen up to 85 gm doses
- Occurrence of lactic acidosis in such circumstances
