Indications
- Treatment of type 2 diabetes mellitus in adults
- Treatment of type 2 diabetes mellitus in children from 10 years of age and adolescents
- Reduction of diabetic complications in overweight type 2 diabetic adult patients
Pharmacology
- Biguanide type oral antihyperglycemic drug
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage And Administration
- Usual starting dose for adults: 500 mg twice a day or 850 mg once a day
- Maximum daily doses for adults: 2000 mg per day (immediate release)
- Dosage increases in increments of 500 mg weekly or 850 mg every 2 weeks
- Usual starting dose for children: 500 mg twice a day
- Maximum daily doses for children: 2000 mg per day
Interaction
- Enhanced absorption when co-administered with Nifedipine
- Potential interaction with cationic drugs eliminated by renal tubular secretion
- No effect on Cimetidine pharmacokinetics
- Certain drugs may lead to loss of glycemic control
Contraindications
- Hypersensitivity to active substance or excipients
- Acute metabolic acidosis
- Severe renal failure (GFR <30 mL/min)
- Acute conditions altering renal function
- Conditions causing tissue hypoxia
Side Effects
- Hemolytic anemia
- Lactic acidosis
- Decrease of vitamin B12 absorption
- Peripheral neuropathy in patients with vitamin B12 deficiency
- Taste disturbance
- Encephalopathy
- Gastrointestinal disorders
- Liver function test abnormalities
- Skin reactions
Pregnancy And Lactation
- Uncontrolled diabetes during pregnancy associated with increased risk of congenital abnormalities and perinatal mortality
- Metformin excreted into human breast milk
- Breastfeeding not recommended during treatment
Precautions And Warnings
- Substantially excreted by the kidney
- Decreased vitamin B12 level
- Increased risk of hypoglycemia when used in combination with insulin
- Regular monitoring of renal function advised
- Therapy should be stopped before surgery
- Not advised in conditions causing dehydration or serious infections
- Regular monitoring of Vitamin B12 levels during continuous therapy
Use In Special Populations
- Adjusted dosage for elderly based on renal function
- No effect on growth and puberty detected in controlled clinical studies
- Caution recommended when prescribing to children aged between 10 and 12 years
Overdose Effects
- Hypoglycemia not seen with doses up to 85 gm
- Lactic acidosis a medical emergency
