Indications
- Adjunct to diet and exercise
- Improves glycemic control
- Type 2 diabetes mellitus treatment
- Not for use in type 1 diabetes
- Not for treatment of diabetic ketoacidosis
- Not studied in patients with history of pancreatitis
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
- Inhibits DPP-8 and DPP-9 activity
- Selectivity for DPP-4
Dosage & Administration
- Recommended dose: 50mg twice a day or 100mg once daily
- Can be taken with or without food
Interaction
- No meaningful alteration in pharmacokinetics of other drugs
- Slight increase in mean Digoxin concentration
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia risk in combination with sulfonylurea and insulin
Pregnancy & Lactation
- Pregnancy Category B
- Caution advised during lactation
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment in renal insufficiency
- Risk of hypoglycemia with medications known to cause it
- Risk of serious hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- Dose adjustment in elderly patients with decreased renal function
Overdose Effects
- Maximal mean increase in QTc of 8.0 msec observed at high doses
- Usual supportive measures in case of overdose
- Modestly dialyzable
