Indications
- Adjunct to diet and exercise for adults with type 2 diabetes mellitus
Pharmacology
- DPP-4 inhibitor
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
Dosage & Administration
- Recommended dose of 50mg twice daily or 100mg once daily
- Can be taken with or without food
Interaction
- Did not meaningfully alter the pharmacokinetics of certain drugs
- Slightly increases the mean concentration of Digoxin
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions include headache, upper respiratory tract infection, and nasopharyngitis
- Hypoglycemia may occur in patients treated with combination to SGN and sulfonylurea
Pregnancy & Lactation
- Pregnancy Category B
- Should be used during pregnancy only if clearly needed
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustments in patients with renal insufficiency
- Use with medications known to cause hypoglycemia requires dosage adjustment
Use in Special Populations
- Safety and effectiveness in pediatric patients under 18 years of age not established
- Careful dose selection in elderly patients with decreased renal function
- Different dosage adjustments for patients with varying degrees of renal insufficiency
Overdose Effects
- Maximal mean increases in QTc of 8.0 msec observed at high doses
- Usual supportive measures should be employed in the event of an overdose
- SGN is modestly dialyzable
