Indications
- Monotherapy and Combination Therapy
- Adjunct to diet and exercise
- Improve glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Regulates glucose homeostasis
- Increases insulin release
- Decreases glucagon levels
- Selectivity for DPP-4
- Does not inhibit DPP-8 or DPP-9 activity
Dosage & Administration
- Recommended dose of 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- Did not meaningfully alter the pharmacokinetics of certain drugs
- Slightly increases the mean concentration of Digoxin
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions include headache, upper respiratory tract infection, and nasopharyngitis
- Hypoglycemia may occur in patients treated with the combination of Sigtil and sulfonylurea, and add on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Secreted in the milk of lactating rats
- Caution advised when administered to nursing women
Precautions & Warnings
- Discontinue if pancreatitis is suspected
- Dosage adjustment recommended in patients with renal insufficiency
- Use with medications known to cause hypoglycemia may require dosage adjustment of sulfonylurea or insulin
- Hypersensitivity reactions reported
- Safety and effectiveness not established in pediatric patients under 18 years of age
- Careful dose selection and renal function assessment advised for elderly patients

Overdose Effects
- Maximal mean increases in QTc observed
- No experience with doses above 800 mg in clinical studies
- Appropriate supportive measures in the event of overdose
