Indications
- Adjunct to diet and exercise
- Improves glycemic control
- Type 2 diabetes mellitus
- Not indicated for type 1 diabetes
Pharmacology
- DPP-4 inhibitor
- Slows inactivation of incretin hormones
- Increases and prolongs action of incretin hormones
- Incretin hormones: GLP-1, GIP
- Regulates glucose homeostasis
- Increases insulin release
- Decreases glucagon levels
Dosage & Administration
- Recommended dose: 50mg twice a day or 100mg once daily
- Can be taken with or without food
Interaction
- No meaningful alteration in pharmacokinetics of several drugs
- Slight increase in Digoxin concentration
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia may occur in certain patient groups
Pregnancy & Lactation
- Pregnancy Category B
- Secreted in the milk of lactating rats
- Caution advised for nursing women
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment in patients with renal insufficiency
- Use with medications known to cause hypoglycemia
- Risk of hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- Elderly patients may require dose adjustment
- Different dosage for patients with renal insufficiency
Overdose Effects
- Maximal mean increases in QTc observed at high dose
- Employ supportive measures in case of overdose
- Modestly dialyzable
