Indications

  • Adjunct to diet and exercise
  • Improves glycemic control
  • Type 2 diabetes mellitus
  • Limitations of use in type 1 diabetes
  • Not for treatment of diabetic ketoacidosis
  • Not studied in patients with history of pancreatitis

Pharmacology

  • DPP-4 inhibitor
  • Slows inactivation of incretin hormones
  • Increases and prolongs action of incretin hormones
  • Involvement in physiologic regulation of glucose homeostasis
  • Selective for DPP-4

Dosage & Administration

  • Recommended dose of 50mg twice a day or 100mg once daily
  • Can be taken with or without food

Interaction

  • No meaningful alteration of pharmacokinetics with specified drugs
  • Slightly increases mean Digoxin concentration
  • No dose adjustment of Digoxin required

Contraindications

  • History of serious hypersensitivity reaction to sitagliptin

Side Effects

  • Common adverse reactions include headache, upper respiratory tract infection, nasopharyngitis
  • Hypoglycemia in combination with sulfonylurea and insulin

Pregnancy & Lactation

  • Pregnancy Category B
  • Reproduction studies performed in rats and rabbits
  • Should be used during pregnancy only if clearly needed
  • Caution advised during lactation

Precautions & Warnings

  • Prompt discontinuation if pancreatitis is suspected
  • Dosage adjustment in patients with renal insufficiency
  • Caution with medications known to cause hypoglycemia
  • Hypersensitivity reactions reported
