Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Not to be used in patients with type 1 diabetes
Pharmacology
- DPP-4 inhibitor
- Increases insulin release and decreases glucagon levels
- Selectivity for DPP-4
Dosage & Administration
- Recommended dose: 50 mg twice a day or 100 mg once daily
- Can be taken with or without food
Interaction
- No meaningful alteration of pharmacokinetics for certain drugs
- Slight increase in Digoxin concentration
Contraindications
- History of serious hypersensitivity reaction to sitagliptin
Side Effects
- Common adverse reactions: headache, upper respiratory tract infection, nasopharyngitis
- Hypoglycemia risk with combination to sulfonylurea and add on to insulin
Pregnancy & Lactation
- Pregnancy Category B
- Caution in lactating women
- Secreted in the milk of lactating rats
Precautions & Warnings
- Prompt discontinuation if pancreatitis is suspected
- Dosage adjustment for renal insufficiency
- Caution with medications known to cause hypoglycemia
- Reports of serious hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- Careful dose selection in elderly with decreased renal function
Overdose Effects
- Maximal mean increases in QTc observed with high doses
- Supportive measures in case of overdose
- Modestly dialyzable
