Pharmacology
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- Slows inactivation of incretin hormones
- Incretin hormones: GLP-1, GIP
- Regulation of glucose homeostasis
- Increases insulin release
- Decreases glucagon levels
- Glucose-dependent manner
Dosage & Administration
- 50 mg or 100 mg daily for monotherapy
- 50 mg twice daily with Metformin or Thiazolidinedione
- 50 mg once daily with Sulphonylurea
- May be taken with or without a meal
- No dosage adjustment for elderly
- No dosage adjustment for patients with mild renal impairment
Interaction
- No interactions with pioglitazone, metformin, glibenclamide, digoxin, warfarin, amlodipine, ramipril, valsartan, simvastatin
- Glucose-lowering effect may be reduced by thiazides, corticosteroids, thyroid products, sympathomimetics
Contraindications
- Hypersensitivity to the active substance or excipients
- Moderate to severe renal impairment
- Hepatic impairment: ALT or AST >3 times ULN
- Type 1 diabetes
Side Effects
- Majority of adverse reactions are mild and transient
- Rare case of hepatic dysfunction
- No additional safety signals in long-term trials
Pregnancy & Lactation
- Not to be used in pregnancy
- Not to be used during lactation
Precautions & Warnings
- Caution in patients aged 75 years and older
- Monitoring of LFTs prior to initiation
- Monitoring of LFTs at three-monthly intervals in the first year
- Use with caution in congestive heart failure patients
- Do not use in NYHA functional class III IV
