Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Improves glycemic control
- Can be used as monotherapy
- Can be used in dual combination with other antidiabetic agents
Pharmacology
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- Slows inactivation of incretin hormones
- Increases insulin release and decreases glucagon levels
- Regulates glucose homeostasis
Dosage & Administration
- Recommended dose of 50mg or 100mg daily for monotherapy
- Different dosage when used in dual combination with other antidiabetic agents
- May be taken with or without a meal
- No dosage adjustment required for elderly or patients with mild renal impairment
Interaction
- No interactions seen with specific medications
- Glucose-lowering effect may be reduced by certain active substances
Contraindications
- Hypersensitivity to the active substance or excipients
- Moderate to severe renal impairment
- Hepatic impairment with elevated liver enzymes
- Type 1 diabetes
Side Effects
- Majority of adverse reactions are mild and transient
- Rare case of hepatic dysfunction
- No additional safety signals in long-term clinical trials
Pregnancy & Lactation
- Not recommended for use during pregnancy
- Not recommended for use during lactation
Precautions & Warnings
- Caution in patients aged 75 years and older
- LFT monitoring prior to initiation and periodically thereafter
- Discontinue treatment if transaminase levels are significantly increased
- Do not use in patients with congestive heart failure NYHA class III-IV
