Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Monotherapy for type 2 diabetes mellitus
- Dual combination therapy with Metformin, Sulphonylurea, or Thiazolidinedione
Pharmacology
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- Inhibition of incretin hormone inactivation
- Increase in insulin synthesis and release
- Decrease in hepatic glucose production
Dosage & Administration
- Recommended daily doses: 50 mg or 100 mg for monotherapy
- Recommended daily doses: 50 mg twice daily for dual combination with Metformin or Thiazolidinedione
- Recommended daily dose: 50 mg once daily in the morning for dual combination with a Sulphonylurea
- Can be taken with or without a meal
- No dosage adjustment for elderly or patients with mild renal impairment
Interaction
- No interactions seen with pioglitazone, metformin, glibenclamide, digoxin, warfarin, amlodipine, ramipril, valsartan, or simvastatin
- Possible reduction in glucose-lowering effect with certain active substances like thiazides, corticosteroids, thyroid products, and sympathomimetics
Contraindications
- Hypersensitivity to the active substance or excipients
- Moderate to severe renal impairment
- Hepatic impairment with elevated ALT or AST levels
- Type 1 diabetes
Side Effects
- Majority of adverse reactions are mild and transient
- Rare cases of hepatic dysfunction
- No additional safety signals found in long-term trials
Pregnancy & Lactation
- Not recommended for use during pregnancy
- Not recommended for use during lactation
Precautions & Warnings
- Caution in patients aged 75 years and older
- Recommended liver function test monitoring
- Discontinuation if transaminase levels are increased
- Caution in patients with congestive heart failure
- Avoid use in patients with NYHA functional class III or IV
