Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Can be used in monotherapy or in dual combination with other antidiabetic agents
Pharmacology
- Dipeptidyl peptidase-4 (DPP-4) inhibitor
- Slows inactivation of incretin hormones
- Increases insulin release and decreases glucagon levels in a glucose-dependent manner
Dosage & Administration
- Recommended dose of 50 mg or 100 mg daily for monotherapy
- 50 mg twice daily for dual combination with Metformin or Thiazolidinedione
- 50 mg once daily for dual combination with a Sulphonylurea
- May be taken with or without a meal
Interaction
- No interactions with specific medications observed in pharmacokinetic studies
- Glucose-lowering effect may be reduced by certain active substances like thiazides and corticosteroids
Contraindications
- Hypersensitivity to active substance or excipients
- Moderate to severe renal impairment
- Hepatic impairment (ALT or AST >3 times the upper limit of normal)
- Type 1 diabetes
Side Effects
- Majority of adverse reactions are mild and transient
- Rare case of hepatic dysfunction reported
- Long-term trials did not reveal additional safety signals
Pregnancy & Lactation
- Not recommended for use during pregnancy
- Not recommended for use during lactation
Precautions & Warnings
- Caution in patients aged 75 years and older
- Liver function tests should be monitored prior to initiation and at regular intervals
- Caution in patients with congestive heart failure of NYHA functional class I-II
- Not recommended in patients with NYHA functional class III-IV
