Indications
- Lowering blood glucose in patients with type 2 diabetes mellitus
- Adjunct to diet and exercise for controlling hyperglycemia
- Use in combination with Metformin for lowering blood glucose
Description
- Oral blood-glucose-lowering drug of the meglitinide class
Pharmacology
- Binds to specific receptors in the cell membrane
- Leads to closure of ATP dependent K⁺ channels
- Stimulates insulin secretion
Dosage & Administration
- Starting dose of 0.5 mg for patients not previously treated
- Initial dose of 1 or 2 mg for patients previously treated
- Should be taken immediately or up to 30 minutes before each meal
- Dosage adjustment according to response
- Maximum 16 mg daily
Interaction
- Possible interactions with cytochrome P450 enzyme system inhibitors
- Possible interactions with cytochrome P450 enzyme system inducers
- Cimetidine has no significant effect on absorption or clearance
- No significant effect on Digoxin, Theophyllin, or Warfarin
- Possible increase in risk of hypoglycemia with co-administration of NSAIDs
- Risk of hypoglycemia may be increased with certain drugs such as salicylates
Contraindications
- Diabetic ketoacidosis, with or without coma
- Type 1 diabetes mellitus
- Known hypersensitivity to the drug or its inactive ingredients
Side Effects
- Hypoglycemia and related symptoms
- Upper respiratory tract infections
- Diarrhea
- Constipation
- Nausea and vomiting
- Hypersensitivity reactions like rashes and urticaria
Pregnancy & Lactation
- Safety in pregnant women not established
- Use during pregnancy only if clearly needed
- Unknown excretion in human milk
- Decision should be made regarding nursing or discontinuation of the drug
Precautions & Warnings
- Insulin substitution during concurrent illness and surgery
- Capable of producing hypoglycemia
- Administration with meals to reduce risk of hypoglycemia
Overdose Effects
- Few adverse effects other than lowering of blood glucose with high doses
- Severe hypoglycemic reactions occur infrequently
