Indications
- Adjunct to diet and exercise

Description
- Oral blood-glucose-lowering drug
- Meglitinide class
- Rapid onset of action and rapid elimination
Pharmacology
- Binds to specific receptors in the cell membrane
- Closure of ATP dependent K+ channels
- Stimulation of insulin secretion
Dosage & Administration
- Starting dose of 0.5 mg before each meal
- Dosage adjustment according to response
- Maximum 16 mg daily
Interaction
- Lower Gluretor clearance and longer half-life with inhibitors of the cytochrome P450 enzyme system
- Accelerated metabolism with inducers of the cytochrome P450 enzyme system
- No significant effect on Digoxin, Theophyllin, or Warfarin
Contraindications
- Diabetic ketoacidosis
- Type 1 diabetes mellitus
- Known hypersensitivity to the drug or its inactive ingredients
Side Effects
- Hypoglycemia and related symptoms
- Upper respiratory tract infections
- Hypersensitivity reactions like rashes and urticaria
Pregnancy & Lactation
- Safety in pregnant women not established
- Not known whether excreted in human milk
- Decision should be made whether to discontinue nursing or the drug
Precautions & Warnings
- Substitute insulin during concurrent illness
- Administer with meals to lessen the risk of hypoglycemia
Overdose Effects
- Lowering of blood glucose with high doses
- Severe hypoglycemic reactions occur infrequently
