Indications
- Adjunct to diet and exercise for lowering blood glucose in type 2 diabetes mellitus (NIDDM)
- Indicated for use in combination with Metformin
- Lower blood glucose in patients not controlled by exercise and diet alone
Description
- Oral blood-glucose-lowering drug
- Belongs to the meglitinide class
- Management of Type 2 diabetes mellitus (NIDDM)
Pharmacology
- Binds to specific receptors in the cell membrane
- Leads to the closure of ATP dependent K+ channels
- Stimulates insulin secretion
- Causes pancreas to release more insulin into the bloodstream
Dosage & Administration
- Starting dose should be 0.5 mg before each meal for some patients
- Initial dose is 1 or 2 mg before each meal for others
- Should be taken immediately or up to 30 minutes before each meal
- Dosage adjustment should be according to response at intervals of 1-2 weeks
Interaction
- Possible interactions with inhibitors and inducers of the cytochrome P450 enzyme system
- No significant effect on Digoxin, Theophyllin, or Warfarin
- Increased risk of hypoglycemia when co-administered with certain drugs
Contraindications
- Contraindicated in diabetic ketoacidosis
- Contraindicated in type 1 diabetes mellitus
- Contraindicated in known hypersensitivity to the drug or its inactive ingredients
Side Effects
- Most common side effects include hypoglycemia, upper respiratory tract infections, diarrhea, constipation, nausea, vomiting
- Hypersensitivity reactions include rashes and urticaria
Pregnancy & Lactation
- Safety in pregnant women not established
- Should be used during pregnancy only if clearly needed
- Not known whether Repaglinide is excreted in human milk
- A decision should be made whether to discontinue nursing or the drug
Precautions & Warnings
- Insulin should be substituted during concurrent illness
- All oral blood glucose-lowering drugs can produce hypoglycemia
- Should be administered with meals to lessen the risk of hypoglycemia
Overdose Effects
- Patients receiving high doses developed few adverse effects other than lowering of blood glucose
- Severe hypoglycemic reactions with coma, seizure or other neurological impairment occur infrequently
