Indications

  • Adjunct to diet and exercise for lowering blood glucose in patients with type 2 diabetes mellitus
  • Use in combination with Metformin to lower blood glucose
  • For patients with hyperglycemia not controlled by diet and exercise alone

Description

  • Oral blood-glucose-lowering drug of the meglitinide class
  • Works by causing pancreas to release more insulin into the bloodstream
  • Rapid onset of action and rapid elimination

Pharmacology

  • Binds to specific receptors in the cell membrane
  • Leads to the closure of ATP dependent K+ channels
  • Depolarisation of cell membrane
  • Ca++ influx and increased intracellular Ca++
  • Stimulation of insulin secretion

Dosage & Administration

  • Starting dose of 0.5 mg for certain patients
  • Initial dose of 1 or 2 mg for previously treated patients
  • Taken immediately or up to 30 minutes before each meal
  • Dosage should be adjusted according to response at intervals of 1-2 weeks
  • Maximum 16 mg daily

Interaction

  • May need dosage adjustment when taken with other medications
  • Possible interactions with inhibitors or inducers of the cytochrome P450 enzyme system
  • Interaction with Cimetidine, Digoxin, Theophyllin, Warfarin, NSAIDs, salicylates, sulphonamides, Chloramphenicol, coumarins, Probenecid, monoamine oxidase (MAO) inhibitors, adrenergic blockers

Contraindications

  • Diabetic ketoacidosis
  • Type 1 diabetes mellitus
  • Known hypersensitivity to the drug or its inactive ingredients

Side Effects

  • Hypoglycemia and related symptoms
  • Upper respiratory tract infections
  • Diarrhea
  • Constipation
  • Nausea and vomiting
  • Hypersensitivity reactions like rashes and urticaria

Pregnancy & Lactation

  • Safety in pregnant women not established
  • Use during pregnancy only if clearly needed
  • Not known whether it is excreted in human milk
  • Decision should be made whether to discontinue nursing or the drug

Precautions & Warnings

  • Insulin should be substituted during concurrent illness and surgery
  • Capable of producing hypoglycemia
  • Administered with meals to lessen the risk of hypoglycemia

Overdose Effects

  • Patients receiving up to 80 mg developed few adverse effects other than lowering of blood glucose
  • Severe hypoglycemic reactions with coma, seizure or other neurological impairment occur infrequently
