Indications
- Adjunct to diet and exercise for lowering blood glucose in type 2 diabetes mellitus patients
- Used in combination with Metformin to lower blood glucose
Description
- Oral blood-glucose-lowering drug of the meglitinide class
- Works by causing pancreas to release more insulin into the blood stream
Pharmacology
- Binds to specific receptors in the cell membrane leading to the closure of ATP dependent K+ channels and depolarization of cell membrane
- Leads to Ca++ influx and increased intracellular Ca++ resulting in stimulation of insulin secretion
Dosage & Administration
- Starting dose of 0.5 mg for patients not previously treated or with HbA1c <8%
- Initial dose of 1 or 2 mg for patients previously treated with higher HbA1c
- Should be taken immediately or up to 30 minutes before each meal
Interaction
- Possible interactions with inhibitors and inducers of the cytochrome P450 enzyme system
- Impact on Digoxin, Theophyllin, Warfarin, and NSAIDs
Contraindications
- Diabetic ketoacidosis
- Type 1 diabetes mellitus
- Known hypersensitivity to the drug or its inactive ingredients
Side Effects
- Hypoglycemia and related symptoms
- Upper respiratory tract infections
- Diarrhea
- Constipation
- Nausea and vomiting
- Hypersensitivity reactions include rashes and urticaria
Pregnancy & Lactation
- Safety in pregnant women has not been established
- Should be used during pregnancy only if clearly needed
- Not known whether it is excreted in human milk
Precautions & Warnings
- Insulin should be substituted during concurrent illness
- Administration with meals to lessen the risk of hypoglycemia
Overdose Effects
- Patients receiving up to 80 mg developed few adverse effects other than lowering of blood glucose
- Severe hypoglycemic reactions occur infrequently
