Indications

  • Adjunct to diet and exercise for lowering blood glucose in patients with type 2 diabetes mellitus
  • Use in combination with Metformin for lowering blood glucose

Description

Oral blood-glucose-lowering drug of the meglitinide class used in the management of Type 2 diabetes mellitus

Pharmacology

  • Binds to specific receptors in the cell membrane
  • Leads to closure of ATP dependent K+ channels and depolarization of cell membrane
  • Leads to increased intracellular Ca++ and stimulation of insulin secretion

Dosage & Administration

  • Starting dose of 0.5 mg for patients not previously treated or with HbA1c <8%
  • Initial dose of 1 or 2 mg for patients previously treated with blood glucose-lowering drugs and HbA1c >8%
  • Taken immediately or up to 30 minutes before each meal
  • Dosage adjusted according to response at intervals of 1-2 weeks

Interaction

  • Inhibitors of cytochrome P450 enzyme system may lead to lower clearance and longer half-life
  • Inducers of cytochrome P450 enzyme system may accelerate metabolism
  • Cimetidine has no significant effect on absorption or clearance
  • No significant effect on certain drugs like Digoxin, Theophyllin, or Warfarin
  • Increased risk of hypoglycemia when co-administered with certain drugs such as salicylates, sulphonamides, Chloramphenicol, coumarins, Probenecid, monoamine oxidase (MAO) inhibitors, and adrenergic blockers

Contraindications

  • Diabetic ketoacidosis
  • Type 1 diabetes mellitus
  • Known hypersensitivity to the drug or its inactive ingredients

Side Effects

  • Hypoglycemia and related symptoms
  • Upper respiratory tract infections
  • Diarrhea
  • Constipation
  • Nausea and vomiting
  • Hypersensitivity reactions like rashes and urticaria

Pregnancy & Lactation

  • Safety during pregnancy not established
  • Should be used during pregnancy only if clearly needed
  • Not known whether excreted in human milk
  • Decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother

Precautions & Warnings

  • Insulin should be substituted during concurrent illness and surgery
  • All oral blood glucose-lowering drugs are capable of producing hypoglycemia
  • Should be administered with meals to lessen the risk of hypoglycemia

Overdose Effects

  • Patients receiving up to 80 mg developed few adverse effects other than lowering of blood glucose
  • Severe hypoglycemic reactions occur infrequently
