Indications

  • Adjunct to diet and exercise for lowering blood glucose in type 2 diabetes mellitus
  • Combination use with Metformin for lowering blood glucose
  • Patients with hyperglycemia not controlled by diet and exercise alone

Description

  • Oral blood-glucose-lowering drug
  • Meglitinide class
  • Rapid onset of action and rapid elimination
  • Manages Type 2 diabetes mellitus (NIDDM)

Pharmacology

  • Binds to specific receptors in cell membrane
  • Causes closure of ATP dependent K channels
  • Stimulates insulin secretion
  • Leads to increased intracellular Ca

Dosage & Administration

  • Starting dose for patients not previously treated or HbA1c < 8%: 0.5 mg before each meal
  • Initial dose for patients previously treated with HbA1c > 8%: 1 or 2 mg before each meal
  • Taken immediately or up to 30 minutes before each meal
  • Dosage adjusted according to response; maximum 16 mg daily

Interaction

  • Possible interactions with azole antifungals and macrolides
  • Accelerated metabolism with Rifampin, Phenobarbital, Carbamazepine, Troglitazone
  • No significant effect on Cimetidine, Digoxin, Theophyllin, or Warfarin
  • Increased plasma level with NSAIDs, leading to potential hypoglycaemia risk
  • Increased risk of hypoglycaemia when co-administered with certain drugs like salicylates, MAO inhibitors, and adrenergic blockers

Contraindications

  • Diabetic ketoacidosis
  • Type 1 diabetes mellitus
  • Known hypersensitivity to the drug or its inactive ingredients

Side Effects

  • Hypoglycemia and related symptoms
  • Upper respiratory tract infections
  • Diarrhea, constipation, nausea, vomiting
  • Hypersensitivity reactions like rashes and urticaria

Pregnancy & Lactation

  • Safety not established in pregnant women
  • Use during pregnancy only if clearly needed
  • Unknown excretion in human milk

Precautions & Warnings

  • Substitute with insulin during concurrent illness and surgery
  • Capable of producing hypoglycemia
  • Administered with meals to reduce the risk of hypoglycemia

Overdose Effects

  • Severe hypoglycemic reactions infrequent
  • Few adverse effects at high doses up to 80 mg
  • Hypoglycemia did not occur with meals given at high doses
