Indications

  • Adjunct to diet and exercise to lower blood glucose in type 2 diabetes mellitus (NIDDM)
  • Indicated for use in combination with Metformin

Description

  • Oral blood-glucose-lowering drug
  • Belongs to the meglitinide class
  • Works by causing pancreas to release more insulin
  • Rapid onset of action and rapid elimination

Pharmacology

  • Binds to specific receptors in the cell membrane
  • Leads to closure of ATP dependent K+ channels
  • Results in depolarisation of cell membrane
  • Causes Ca++ influx and increased intracellular Ca++
  • Stimulates insulin secretion

Dosage & Administration

  • Starting dose varies based on HbA1c levels
  • Should be taken immediately or up to 30 minutes before each meal
  • Dosage should be adjusted according to response
  • Maximum 16 mg daily

Interaction

  • Potential interactions with inhibitors and inducers of cytochrome P450 enzyme system
  • Cimetidine has no significant effect
  • No significant effect on Digoxin, Theophylline, or Warfarin
  • Potential interaction with highly protein bound drugs
  • Increased risk of hypoglycemia with certain co-administered drugs

Contraindications

  • Diabetic ketoacidosis, with or without coma
  • Type 1 diabetes mellitus
  • Known hypersensitivity to the drug or its inactive ingredients

Side Effects

  • Most common are hypoglycemia and related symptoms
  • Other side effects include upper respiratory tract infections, diarrhea, constipation, nausea, vomiting, rashes, and urticaria

Pregnancy & Lactation

  • Safety in pregnant women has not been established
  • Use during pregnancy only if clearly needed
  • Not known whether excreted in human milk
  • Consideration should be made for nursing infants

Precautions & Warnings

  • Insulin should be substituted during concurrent illness and surgery
  • All oral blood-glucose-lowering drugs can produce hypoglycemia
  • Should be administered with meals to lessen the risk of hypoglycemia

Overdose Effects

  • Patients receiving up to 80 mg developed few adverse effects other than lowering of blood glucose
  • Severe hypoglycemic reactions occur infrequently
