Indications

  • Adjunct to diet and exercise for type II diabetes
  • Monotherapy and combination therapy indication
  • Improves glycaemic control
  • Use in combination with sulphonylurea, Metformin or Insulin

Pharmacology

  • Belongs to thiazolidinediones class
  • Depends on the presence of Insulin for mechanism of action
  • Decreases Insulin resistance in the periphery and liver
  • Activates peroxisome proliferator activated receptor gamma (PPAR-γ)

Dosage & Administration

  • Can be taken once daily without regard to meals
  • Individualized management
  • Initiate at 15 mg or 30 mg once daily for monotherapy
  • Maximum recommended daily dose is 45 mg
  • Consider combination therapy for inadequate response

Interaction

  • Reduces the plasma concentration of oral contraceptives containing ethinyl oestradiol and norethindrone

Contraindications

  • Contraindicated in patients with known hypersensitivity to any components

  • Incidence of upper respiratory tract infection, headache, sinusitis, myalgia, tooth disorder, and pharyngitis

Pregnancy & Lactation

  • Use during pregnancy if potential benefit justifies potential risk to the foetus
  • Unknown secretion in human milk
  • Should not be administered to lactating women

Precautions & Warnings

  • Exerts antihyperglycaemic effect only in the presence of Insulin
  • Should not be used in Type 1 diabetes or diabetic ketoacidosis
  • Use with caution in case of combination antidiabetic therapy and hepatic insufficiency
  • Regular monitoring of liver enzymes
