Indications
- Improves glycaemic control in patients with type II diabetes (NIDDM)
- Indicated for monotherapy
- Indicated for use in combination with sulphonylurea, Metformin or Insulin
Pharmacology
- Belongs to thiazolidinediones class
- Depends on the presence of Insulin
- Decreases Insulin resistance in the periphery and in the liver
- Increases Insulin dependent glucose disposal
- Decreases hepatic glucose output
- Activates peroxisome proliferator activated receptor gamma (PPAR-γ)
Dosage & Administration
- Can be taken once daily without regard to meals
- Initiated at 15 mg or 30 mg once daily dosages
- Maximum recommended daily dose should not exceed 45 mg
Interaction
- Reduces plasma concentration of an oral contraceptive containing ethinyl oestradiol and norethindrone by approximately 30%
Contraindications
- Contraindicated in patients with known hypersensitivity to any of its components
Side Effects
- Incidence and types of adverse events reported are upper respiratory tract infection, headache, sinusitis, myalgia, tooth disorder, and pharyngitis
Pregnancy & Lactation
- Not enough studies in pregnant women
- Should be used during pregnancy only if the potential benefit justifies the potential risk to the foetus
- Not known whether Pioglitazone is secreted in human milk
- Should not be administered to a lactating women
Precautions & Warnings
- Not to be used in Type 1 diabetes or for the treatment of diabetic ketoacidosis
- Should be used with caution in case of combination antidiabetic therapy and hepatic insufficiency
- Liver enzyme should be monitored regularly
