Indications
- Adjunct to diet and exercise
- Improves glycaemic control in type II diabetes (NIDDM)
- Monotherapy
- Combination therapy with sulphonylurea, Metformin, or Insulin
Pharmacology
- Member of thiazolidinediones class
- Depends on the presence of Insulin for mechanism of action
- Decreases Insulin resistance
- Increases Insulin dependent glucose disposal
- Decreases hepatic glucose output
- Activates peroxisome proliferator activated receptor gamma (PPAR-γ)
- Improves abnormality in lipid metabolism
Dosage & Administration
- Can be taken once daily
- No regard to meals
- Individualized therapy management
- Initiate with 15 mg or 30 mg once daily dosages
- Maximum recommended daily dose is 45 mg
- Consider combination therapy for inadequate response
Interaction
- Reduces plasma concentration of oral contraceptives containing ethinyl oestradiol and norethindrone
Contraindications
- Known hypersensitivity to any components
Side Effects
- Incidence and types of adverse events reported in clinical trials
- Specific adverse events include upper respiratory tract infection, headache, sinusitis, myalgia, tooth disorder, pharyngitis
Pregnancy & Lactation
- Use during pregnancy if potential benefit justifies potential risk
- Not known if secreted in human milk
- Not recommended for lactating women
Precautions & Warnings
- Not to be used in Type 1 diabetes or for the treatment of diabetic ketoacidosis
- Caution in combination antidiabetic therapy and hepatic insufficiency
- Regular monitoring of liver enzymes
