Indications
- Improves glycaemic control in type II diabetes
- Adjunct to diet and exercise
- Can be used in combination therapy
Pharmacology
- Member of thiazolidinediones
- Depends on presence of Insulin for mechanism of action
- Decreases Insulin resistance in periphery and liver
- Activates peroxisome proliferator activated receptor gamma (PPAR-γ)
- Improves abnormality in lipid metabolism
Dosage & Administration
- Taken once daily without regard to meals
- Individualized management
- Initiated at 15 mg or 30 mg once daily for monotherapy
- Can be increased up to 45 mg once daily
- Maximum recommended daily dose should not exceed 45 mg
Interaction
- Reduces plasma concentration of oral contraceptives containing ethinyl oestradiol and norethindrone
Contraindications
- Known hypersensitivity to any components
Side Effects
- Reported adverse events include upper respiratory tract infection, headache, sinusitis, myalgia, tooth disorder, pharyngitis
Pregnancy & Lactation
- Use during pregnancy if benefit justifies risk to the fetus
- Not known if secreted in human milk, should not be administered to lactating women
Precautions & Warnings
- Exerts effect only in presence of Insulin
- Should not be used in Type 1 diabetes or for diabetic ketoacidosis
- Caution in case of combination antidiabetic therapy and hepatic insufficiency
- Regular monitoring of liver enzymes
