Indications
- Adjunct to diet and exercise for type II diabetes control
- Monotherapy for type II diabetes
- Combination therapy with sulphonylurea, Metformin, or Insulin
Pharmacology
- Mechanism of action dependent on presence of Insulin
- Decreases Insulin resistance in the periphery and liver
- Increases Insulin dependent glucose disposal
- Decreases hepatic glucose output
- Improves abnormality in lipid metabolism
Dosage & Administration
- Can be taken once daily without regard to meals
- Initiate at 15 mg or 30 mg once daily for monotherapy
- Maximum recommended daily dose of 45 mg
- Consider combination therapy for inadequate response to monotherapy
Interaction
- Reduces plasma concentration of oral contraceptive containing ethinyl oestradiol and norethindrone by approximately 30%
Contraindications
- Contraindicated in patients with known hypersensitivity to any of its components
Side Effects
- Incidence and types of adverse events reported in clinical trials
- Common adverse events include upper respiratory tract infection, headache, sinusitis, myalgia, tooth disorder, and pharyngitis
Pregnancy & Lactation
- Limited studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to the foetus
- Not known whether Pioglitazone is secreted in human milk
Precautions & Warnings
- Exerts antihyperglycaemic effect only in presence of Insulin
- Not for use in Type 1 diabetes or diabetic ketoacidosis
- Use with caution in combination antidiabetic therapy and hepatic insufficiency
- Monitor liver enzymes regularly
