Description
- Combination therapy for type 2 diabetes
- Improves glycemic control
- Used when hyperglycemia cannot be managed with diet and exercise alone
- Recommended for patients who do not respond to initial treatment with a Sulfonylurea or Metformin
- Pharmacological mechanisms include glucose tolerance improvement, lowered plasma glucose, decreased hepatic glucose production, improved insulin sensitivity, and stimulation of insulin release from the pancreas
- Dosage should be given once or twice daily with meals, initiated at a low dose, and gradually escalated
- Interacts with cimetidine, alcohol, anti-coagulants, nifedipine, nonsteroidal anti-inflammatory agents, azoles, salicylates, sulfonamides, chloramphenicol, probenecid, coumarins, monoamine oxidase inhibitors, and beta adrenergic blocking agents
- Contraindicated in cases of hypersensitivity to the drug, renal impairment, diabetic coma, ketoacidosis, chronic renal diseases, congestive heart failure, trauma, dehydration, alcohol dependence, pregnancy, and breast-feeding
- Common side effects include hypoglycemia, diarrhea, nausea/vomiting, abdominal pain, headache, and musculoskeletal pain
- Contraindicated during pregnancy and lactation
- Metformin is substantially excreted by the kidney and the risk of Metformin accumulation and lactic acidosis is increased with impaired renal function
- Should be carefully titrated in patients with advanced age and reduced renal function
- Should be temporarily suspended for any surgical procedure and not restarted until oral intake has resumed and renal function has been evaluated as normal
- Should be monitored during concomitant therapy with sulfonylurea, especially in hospital until the correct ratio of the two drugs has been established
- Glipizide metabolism and excretion may be slowed in patients with impaired renal and/or hepatic function
