Indications

• Adjunct to diet and exercise for glycemic control in type 2 diabetes
• Improves insulin sensitivity and glucose disposal

Pharmacology

• Decreases insulin resistance
• Modulates transcription of insulin responsive genes
• Improves glucose tolerance
• Lowers basal and postprandial plasma glucose

Dosage and Administration

• Individualized antihyperglycemic therapy
• Initial and maintenance dosing should be conservative in elderly patients

Interaction

• May impair vit B12 absorption
• Increased plasma levels with strong CYP2C8 inhibitors
• Decreased plasma levels with CYP2C8 inducers

Contraindications

• Renal disease or dysfunction
• Known hypersensitivity to components
• Acute or chronic metabolic acidosis

Side Effects

• Upper respiratory tract infection
• Diarrhea
• Peripheral edema
• Headache

Pregnancy and Lactation

• Potential benefit justifies potential risk to fetus
• Should not be administered to breastfeeding women

Precautions and Warnings

• Should not be used in type 1 diabetes or diabetic ketoacidosis
• Used with caution in combination with insulin, hepatic insufficiency, and heart diseases
