Indications

Pharmacology
- Pioglitazone mechanism of action dependent on insulin presence
- Decreases insulin resistance in periphery and liver
- Increased insulin-dependent glucose disposal
- Selective agonist for peroxisome proliferator-activated receptor-gamma (PPARg)
- Metformin improves glucose tolerance in type 2 diabetes patients
- Decreases hepatic glucose production
- Increases peripheral glucose uptake and utilization
- Does not produce hypoglycemia or hyperinsulinemia
Dosage & Administration
- Individualized antihyperglycemic therapy
- Maximum recommended daily dose of Pioglitazone 45 mg and Metformin 2550 mg
- Divided daily doses with meals
- Starting dose based on patient's current regimen
- Conservative dosing for patients with advanced age
- Monitoring renal function in elderly patients
- Liver enzyme monitoring prior to therapy initiation
Interaction
- May impair vit B12 absorption
- Increased plasma levels with strong CYP2C8 inhibitors
- Decreased plasma levels with CYP2C8 inducers
- Reduced metabolism with cationic drugs
Contraindications
- Renal disease or dysfunction
- Known hypersensitivity to Pioglitazone or Metformin
- Acute or chronic metabolic acidosis
Side Effects
- Upper respiratory tract infection
- Diarrhea
- Peripheral edema
- Headache
- Mild in severity
Pregnancy & Lactation
- Limited studies in pregnant women
- Only used if benefit justifies potential risk to the fetus
- Not recommended for breastfeeding women
Precautions & Warnings
- Not for type 1 diabetes or diabetic ketoacidosis treatment
- Caution in combination with insulin, hepatic insufficiency, and heart diseases
- Risk of Metformin accumulation and lactic acidosis with renal impairment
