Therapeutic Class
- Combination Oral hypoglycemic preparations
Dosage & Administration
- Individualized dosage based on patient's regimen
- Maximum recommended daily dose is 100 mg sitagliptin and 2000 mg metformin
- Should generally be given twice daily with meals
- Gradual dose escalation to reduce gastrointestinal side effects
Contraindications
- Renal disease or dysfunction
- Acute or chronic metabolic acidosis
- History of serious hypersensitivity reaction
- Undergoing radiologic studies involving iodinated contrast materials
Indications
- Adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus
- Not for use in patients with type 1 diabetes or diabetic ketoacidosis
- Not studied in patients with history of pancreatitis
Mode of Action
- Combines two antihyperglycemic agents with complementary mechanisms
- Sitagliptin: DPP-4 inhibitor slowing inactivation of incretin hormones
- Metformin HCl: Decreases hepatic glucose production, intestinal absorption of glucose, and increases peripheral glucose uptake
Interaction
- Cationic drugs: Use with caution if eliminated by renal tubular secretion
- Phenprocoumon: Metformin may decrease anticoagulant effect
- Levothyroxine: Can reduce hypoglycemic effect of metformin
Side Effects
- Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
- Hypoglycemia reported in combination therapy with sulfonylurea, insulin, and metformin monotherapy
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Caution when administered to nursing women due to unknown excretion in human milk
Precautions & Warnings
- Lactic Acidosis risk increases with certain conditions
- Regular monitoring of TSH levels and vitamin B12 levels recommended
- Do not use in patients with hepatic disease
- Assess renal function before and during treatment
- Promptly evaluate patients for ketoacidosis or lactic acidosis if clinical illness develops
Overdose Effects
- Supportive measures for sitagliptin overdose
- Metformin overdose may require hemodialysis for drug removal
- Pancreatitis may occur with metformin overdose
