Indications

  • Adjunct to diet and exercise for type 2 diabetes mellitus
  • Improves glycemic control in adults
  • Not for use in type 1 diabetes or diabetic ketoacidosis
  • Not studied in patients with history of pancreatitis

Pharmacology

  • Combines Sitagliptin and Metformin HCl
  • Dipeptidyl peptidase-4 (DPP-4) inhibitor
  • Biguanide class member
  • Slows inactivation of incretin hormones
  • Regulates glucose homeostasis
  • Increases insulin release and decreases glucagon levels
  • Decreases hepatic glucose production

Dosage & Administration

  • Individualized dosage based on regimen, effectiveness, and tolerability
  • Maximum recommended daily dose of 100 mg sitagliptin and 2000 mg metformin
  • Given twice daily with meals
  • Gradual dose escalation to reduce gastrointestinal side effects
  • Specific starting doses for patients not currently treated with metformin and those already treated with metformin
  • Special dosing for extended-release tablet

Interaction

  • Use with caution with cationic drugs
  • May decrease anticoagulant effect of phenprocoumon
  • Levothyroxine can reduce hypoglycemic effect of metformin

Contraindications

  • Renal disease or dysfunction
  • Acute or chronic metabolic acidosis
  • History of serious hypersensitivity reaction

Side Effects

  • Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
  • Other adverse reactions related to different treatment combinations

Pregnancy & Lactation

  • Category B for pregnancy
  • Caution advised during nursing due to potential drug excretion in human milk

Precautions & Warnings

  • Risk of lactic acidosis with specific associated conditions
  • Monitoring of thyroid-stimulating hormone (TSH) levels recommended
  • Decrease in vitamin B12 serum levels with long-term treatment
  • Postmarketing reports of acute renal failure and pancreatitis
  • Regular monitoring of hematologic parameters
  • Patients advised against excessive alcohol intake

  • Metformin overdosage and dialyzable clearance
