Indications
- Adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus
- Not indicated for type 1 diabetes or diabetic ketoacidosis
- Not studied in patients with a history of pancreatitis
Pharmacology
- Combines sitagliptin and metformin
- Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor
- Metformin HCl belongs to the biguanide class
Dosage & Administration
- Individualized dosage based on patient's current regimen, effectiveness, and tolerability
- Maximum recommended daily dose of 100 mg sitagliptin and 2000 mg metformin
- Gradual dose escalation to reduce gastrointestinal side effects
- Twice daily administration with meals
- Starting dose based on patient's current regimen
Interaction
- Cationic drugs eliminated by renal tubular secretion
- Metformin may decrease the anticoagulant effect of phenprocoumon
- Levothyroxine can reduce the hypoglycemic effect of metformin
Contraindications
- Renal disease or dysfunction
- Acute or chronic metabolic acidosis
- History of serious hypersensitivity reaction to the tablet or sitagliptin
- Use should be temporarily discontinued in patients undergoing radiologic studies with iodinated contrast materials
Side Effects
- Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
- Hypoglycemia reported in patients simultaneously started on sitagliptin and metformin
- Nasopharyngitis reported in patients treated with sitagliptin monotherapy
- Common adverse reactions due to initiation of metformin therapy include diarrhea, nausea/vomiting, and headache
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly needed
- Caution advised when administered to a nursing woman
Precautions & Warnings
- Lactic acidosis risk increases with certain conditions
- Monitoring of thyroid-stimulating hormone (TSH) and vitamin B12 levels recommended
- Do not use in patients with hepatic disease
- Regular assessment of renal function and hematologic parameters recommended
- Warn against excessive alcohol intake
- Need for temporary use of insulin during periods of stress and decreased intake of fluids and food
- Prompt evaluation for evidence of ketoacidosis or lactic acidosis
- Consideration of lower dose of insulin secretagogue or insulin when used in combination
Overdose Effects
- In the event of an overdose, employ usual supportive measures
- Sitagliptin is modestly dialyzable
- Metformin is dialyzable with a clearance of up to 170 mL/min
