Indications

  • Adjunct to diet and exercise for glycemic control in type 2 diabetes mellitus
  • Not for type 1 diabetes or diabetic ketoacidosis

Pharmacology

  • Combines sitagliptin and metformin for glycemic control in type 2 diabetes
  • Sitagliptin is DPP-4 inhibitor, Metformin is a biguanide class member

Dosage & Administration

  • Dosage should be individualized based on patient's regimen and tolerability
  • Starting dose varies for patients not currently treated with metformin and those already treated with metformin
  • Extended-release tablet to be administered once daily with a meal

Interaction

  • Caution with cationic drugs eliminated by renal tubular secretion
  • Metformin may decrease anticoagulant effect of phenprocoumon
  • Levothyroxine can reduce hypoglycemic effect of metformin

Contraindications

  • Contraindicated in renal disease or dysfunction, metabolic acidosis, and serious hypersensitivity reaction

Side Effects

  • Common adverse reactions include diarrhea, upper respiratory tract infection, headache
  • Hypoglycemia reported with combination therapy with sulfonylurea/insulin and metformin
  • Nasopharyngitis reported with sitagliptin monotherapy

Pregnancy & Lactation

  • Pregnancy Category B - Safety in pregnant women not known, to be used if clearly needed
  • Caution advised when administered to nursing women

Precautions & Warnings

  • Lactic acidosis risk with metformin accumulation
  • Regular monitoring of thyroid-stimulating hormone levels recommended in patients with hypothyroidism
  • Long-term metformin treatment associated with decrease in vitamin B12 serum levels

Overdose Effects

  • Supportive measures for sitagliptin overdose
  • Hemodialysis may be useful for removal of accumulated metformin in overdosage
