Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
- Not for use in patients with type 1 diabetes or for the treatment of diabetic ketoacidosis
- Not studied in patients with a history of pancreatitis
Pharmacology
- Combines two antihyperglycemic agents with complementary mechanisms of action
- Sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor
- Metformin HCl is a member of the biguanide class
- Sitagliptin slows inactivation of incretin hormones
- Incretins regulate glucose homeostasis
- Metformin decreases hepatic glucose production and increases peripheral glucose uptake
- Metformin decreases intestinal absorption of glucose
Dosage & Administration
- Individualized dosage based on patient's current regimen, effectiveness, and tolerability
- Maximum recommended daily dose of 100 mg sitagliptin and 2000 mg metformin
- Generally given twice daily with meals
- Starting doses for patients not currently treated with metformin and for patients already treated with metformin
- Extended-release tablet administered once daily with a meal
- Adjust dosing based on effectiveness and tolerability
Interaction
- Use caution with cationic drugs eliminated by renal tubular secretion
- Metformin may decrease the anticoagulant effect of phenprocoumon
- Levothyroxine can reduce the hypoglycemic effect of metformin
Contraindications
- Contraindicated in patients with renal disease or dysfunction
- Contraindicated in patients with acute or chronic metabolic acidosis
- Contraindicated in patients with a history of serious hypersensitivity reaction to the tablet or sitagliptin
- Temporarily discontinued in patients undergoing radiologic studies involving intravascular administration of iodinated contrast materials
Side Effects
- Common adverse reactions include diarrhea, upper respiratory tract infection, and headache
- Hypoglycemia reported in patients treated with sitagliptin in combination with sulfonylurea and metformin
- Nasopharyngitis reported in patients treated with sitagliptin monotherapy
- Common adverse reactions due to initiation of metformin therapy include diarrhea, nausea/vomiting, and headache
Pregnancy & Lactation
- Pregnancy Category B
- Safety in pregnant women not known
- Caution when administered to a nursing woman
Precautions & Warnings
- Lactic acidosis can occur due to metformin accumulation
- Regular monitoring of thyroid-stimulating hormone (TSH) levels recommended in patients with hypothyroidism
- Long-term treatment with metformin associated with decrease in vitamin B12 serum levels
- Postmarketing reports of acute renal failure and pancreatitis
- Monitoring of renal function and hematologic parameters recommended
- Patients warned against excessive alcohol intake
- Discontinuation and temporary use of insulin may be required during periods of stress and decreased intake of fluids and food
- Evaluation for evidence of ketoacidosis or lactic acidosis in patients previously controlled on the tablet who develop laboratory abnormalities or clinical illness
- Lower dose of insulin secretagogue or insulin may be required when used with an insulin secretagogue or insulin
- Postmarketing reports of serious allergic and hypersensitivity reactions in patients treated with sitagliptin
- No clinical studies establishing conclusive evidence of macrovascular risk reduction with Sitagliptin Phosphate Monohydrate INN/Metformin Hydrochloride BP or any other anti-diabetic drug
Overdose Effects
- Usual supportive measures recommended in the event of an overdose
- Sitagliptin is modestly dialyzable
- Metformin is dialyzable with a clearance of up to 170 mL/min
