Indications
- adjunct to diet and exercises
- improves glycaemic control

Pharmacology
- inhibiting DPP-4
- increased fasting and postprandial endogenous levels of incretin hormones
- increased insulin/glucagon ratio
- decreased hepatic glucose production
- improved insulin sensitivity
- decreased intestinal absorption of glucose
Dosage & Administration
- initiation based on current Metformin dose
- twice daily dosing
- administration with or just after food
- switching from separate tablets
Interaction
- no clinically relevant pharmacokinetic interactions
- low potential for drug interactions
- no interaction with CYP 450 enzymes
- interaction with other oral antidiabetics
- interaction with specific medications
Contraindications
- known hypersensitivity to Vildagliptin or Metformin Hydrochloride
- renal disease or dysfunction
- acute myocardial infarction
- septicaemia
- congestive heart failure
- acute or chronic metabolic acidosis
- radiologic studies with iodinated contrast materials
Side Effects
- headache
- tremor
- dizziness
- nausea
- hypoglycaemia
Pregnancy & Lactation
- no adequate and well-controlled studies in pregnant women
- potential benefit justifying potential risk to the fetus
- no studies in breastfeeding women
Precautions & Warnings
- risk of lactic acidosis
- monitoring serum creatinine
- special caution in elderly patients
- monitoring Liver Function Tests (LFTs)
- immediate discontinuation in case of liver dysfunction
- discontinuation before elective surgery
Use in Special Populations
- not recommended for pediatric patients
- monitoring renal function in elderly patients
- contraindicated in patients with renal impairment
- not recommended in patients with hepatic impairment
