Therapeutic Class

  • Combination Oral hypoglycemic preparations

Indications

  • Adjunct to diet and exercises to improve glycaemic control in type 2 diabetes mellitus patients

Pharmacology

  • Acts by inhibiting DPP-4 enzyme
  • Increases insulin secretion
  • Decreases glucagon secretion
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage & Administration

  • Initiated at twice daily
  • Recommended to be taken with or just after food

Interaction

  • Low potential for drug interactions
  • No clinically relevant interactions with other oral antidiabetics
  • Increased Cmax and blood AUC of Metformin with certain medications

Contraindications

  • Known hypersensitivity to Vildagliptin or Metformin Hydrochloride
  • Renal disease or dysfunction
  • Acute myocardial infarction
  • Septicaemia
  • Congestive heart failure
  • Acute or chronic metabolic acidosis
  • Use during radiologic studies involving intravascular administration of iodinated contrast materials

Side Effects

  • Headache
  • Tremor
  • Dizziness
  • Nausea
  • Hypoglycaemia

Pregnancy & Lactation

  • Not recommended during pregnancy unless potential benefit justifies potential risk to the foetus
  • Not recommended for breast-feeding women

Precautions & Warnings

  • Risk of lactic acidosis
  • Monitoring of serum creatinine and liver function
  • Special caution in elderly patients
  • Discontinue in case of jaundice or liver dysfunction
  • Discontinue 48 hours before elective surgery with general anaesthesia

Use in Special Populations

  • Not recommended for use in children below 18 years of age
  • Renal function monitoring in elderly patients
  • Not recommended in patients with renal or hepatic impairment
