Indications
- Adjunct to diet and exercises
- Improves glycaemic control
- Type 2 diabetes mellitus
- Inadequately controlled on Metformin Hydrochloride or Vildagliptin alone
- Combination of Vildagliptin and Metformin Hydrochloride
Pharmacology
- Inhibits DPP-4 enzyme
- Increases insulin secretion
- Decreases glucagon secretion
- Decreases hepatic glucose production
- Decreases intestinal absorption of glucose
- Improves insulin sensitivity
Dosage & Administration
- Initiated at twice daily
- Morning and evening dosing
- Switching from separate tablets
- Doses higher than 100 mg of vildagliptin not recommended
- Reduces gastrointestinal symptoms when taken with food
Interaction
- No clinically relevant pharmacokinetic interaction observed
- Low potential for drug interactions
- Not likely to interact with CYP 450 enzyme substrates, inhibitors or inducers
- No clinically relevant interactions with other oral antidiabetics, amlodipine, digoxin, ramipril, simvastatin, valsartan or warfarin
Contraindications
- Known hypersensitivity to Vildagliptin or Metformin Hydrochloride or to any of the excipients
- Renal disease or dysfunction
- Acute myocardial infarction
- Septicaemia
- Congestive heart failure
- Metabolic acidosis including diabetic ketoacidosis
Side Effects
- Headache
- Tremor
- Dizziness
- Nausea
- Hypoglycemia
Pregnancy & Lactation
- Not recommended during pregnancy
- Potential risk to the fetus
- Not known whether excreted in human milk
- Not recommended for breast-feeding women
Precautions & Warnings
- Lactic acidosis risk
- Discontinue treatment if metabolic acidosis suspected
- Monitor serum creatinine regularly
- Caution in elderly patients
- Monitor Liver Function Tests (LFTs) prior to initiation
- Discontinue 48 hours before elective surgery with general anesthesia
Use in Special Populations
- Not recommended for use in pediatric patients
- Monitor renal function in elderly patients
- Not recommended in patients with renal failure
- Not recommended in patients with hepatic impairment
