Purpose
- Adjunct to diet and exercises to improve glycaemic control in patients with type 2 diabetes mellitus
Ingredients
- Vildagliptin
- Metformin Hydrochloride
Mechanism of Action
- Inhibits DPP-4 enzyme
- Inhibition of DPP-4 activity
- Increases insulin secretion
- Decreases glucagon secretion
- Lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
- Increase in peripheral glucose uptake and utilization
- Decreases intestinal absorption of glucose
Dosage Instructions
- Twice daily administration
- May be initiated based on current Metformin dose
- Doses higher than 100 mg of vildagliptin are not recommended
- May be taken with or just after food to reduce gastrointestinal symptoms associated with Metformin
Pharmacokinetic Interaction
- No clinically relevant interaction observed between Vildagliptin and Metformin Hydrochloride
- Low potential for drug interactions
- Does not interact with CYP 450 enzymes
Contraindications
- Known hypersensitivity to Vildagliptin or Metformin Hydrochloride or to any of the excipients
- Renal disease or dysfunction
- Acute myocardial infarction
- Septicaemia
- Congestive heart failure
- Acute or chronic metabolic acidosis including diabetic ketoacidosis
- Use with iodinated contrast materials
Side Effects
- Headache
- Tremor
- Dizziness
- Nausea
- Hypoglycemia
Pregnancy & Lactation
- Not recommended during pregnancy unless potential benefit justifies potential risk to the fetus
- Not recommended for use in breastfeeding women
Precautions & Warnings
- Possible occurrence of lactic acidosis due to Metformin accumulation
- Serum creatinine monitoring
- Special caution in elderly patients
- Liver Function Tests monitoring
- Discontinuation prior to certain medical procedures
Use in Special Populations
- Not recommended for pediatric patients
- Renal function monitoring for elderly patients
- Not recommended in patients with renal or hepatic impairment
