Indications
- Adjunct to diet and exercises
- Improving glycaemic control
- Type 2 diabetes mellitus
Pharmacology
- Inhibition of DPP-4 enzyme
- Increase in incretin hormones GLP-1 and GIP
- Insulin secretion from pancreatic beta cell
- Decrease in glucagon secretion from alpha cell
- Reduction in hepatic glucose production
Dosage & Administration
- Initiation based on current Metformin dose
- Twice daily administration
- Switching from separate tablets
- No clinical experience in triple combination with other antidiabetic agents
- Reduced gastrointestinal symptoms with food intake
Interaction
- No clinically relevant pharmacokinetic interaction observed
- Low potential for drug interactions
- Not a cytochrome P (CYP) 450 enzyme substrate
- No clinically relevant interactions with certain oral antidiabetics and other medications
Contraindications
- Known hypersensitivity to components
- Renal disease or dysfunction
- Acute myocardial infarction
- Septicaemia
- Congestive heart failure
- Metabolic acidosis and diabetic ketoacidosis
- Use before radiologic studies involving intravascular administration of iodinated contrast materials

Pregnancy & Lactation
- No adequate and well controlled studies in pregnant women
- Potential benefit justifying potential risk to the foetus
- Unknown excretion in human milk
Precautions & Warnings
- Lactic acidosis risk
- Serum creatinine monitoring in patients
- Caution in elderly patients and with certain medications
- Liver Function Tests (LFTs) monitoring
- Discontinuation before elective surgery with general anaesthesia
Use in Special Populations
- Not recommended for pediatric patients
- Regular renal function monitoring in elderly patients
- Contraindicated in patients with renal or hepatic impairment
