Indications
- Adjunct to diet and exercises
- Improves glycaemic control
- Type 2 diabetes mellitus
- Not adequately controlled on Metformin Hydrochloride or Vildagliptin alone
- Combination of Vildagliptin and Metformin Hydrochloride
- Rapid and complete inhibition of DPP-4 activity
Pharmacology
- Inhibits DPP-4
- Degradation of incretin hormones GLP-1 and GIP
- Increases insulin secretion
- Decreases glucagon secretion
- Decreases hepatic glucose production
- Lowers basal and postprandial plasma glucose
Dosage & Administration
- Initiated at twice daily
- 1 tablet in the morning and 1 tablet in the evening
- Switch from separate tablets to combination
- No clinical experience in triple combination with other antidiabetic agents
- Should be taken with or just after food
Interaction
- No clinically relevant pharmacokinetic interaction observed
- Low potential for drug interactions
- Not a cytochrome P (CYP) 450 enzyme substrate
- No clinically relevant interactions with other oral antidiabetics
- Increase in Cmax and blood AUC of Metformin by furosemide, nifedipine, and glyburide
Contraindications
- Known hypersensitivity to Vildagliptin or Metformin Hydrochloride
- Renal disease or renal dysfunction
- Acute myocardial infarction
- Septicaemia
- Congestive heart failure
- Acute or chronic metabolic acidosis, including diabetic ketoacidosis
Side Effects
- Headache
- Tremor
- Dizziness
- Nausea
- Hypoglycaemia
Pregnancy & Lactation
- No adequate and well controlled studies in pregnant women
- Should not be used during pregnancy
- Unknown excretion in human milk
- Should not be administered to breast-feeding women
Precautions & Warnings
- Lactic acidosis
- Serum creatinine monitoring
- Special caution in elderly patients
- Liver Function Tests (LFTs) monitoring
- Discontinuation before elective surgery with general anaesthesia
Use in Special Populations
- Not recommended for use in children below 18 years of age
- Renal function monitoring in elderly patients
- Not recommended in patients with renal failure or renal dysfunction
- Not recommended in patients with hepatic impairment
