Indications
- Replacement therapy in hypothyroidism
- Suppression of Thyroid Stimulating Hormone levels
- Suppression of goitrogenic effects of other drugs
- Diagnostic aid in suppression tests
Pharmacology
- Contains synthetic Levothyroxine
- Identical to natural hormone T4
- T4 conversion to T3 in peripheral tissues
- TBG as major carrier of T4
- Long half-life in circulation
- Liver as major site of degradation of Thyroid hormones
Dosage & Administration
- Initial starting dose: 25-50 mcg/day for adults
- Initial dose: 12.5-25 mcg/day for severe hypothyroidism
- Pediatric dosage based on age group
- Dose adjustment based on clinical response and laboratory parameters
Interaction
- Concurrent use of tri/tetracyclic antidepressants may increase therapeutic and toxic effects
- Sertraline may result in increased Euthycin requirements
- Addition of Euthycin to antidiabetic or insulin therapy may result in increased requirements
Contraindications
- Untreated subclinical or overt Thyrotoxicosis
- Acute Myocardial Infarction
- Uncorrected Adrenal failure

- Adverse reactions include those of hyperthyroidism

Pregnancy & Lactation
- Pregnancy Category A
- Caution required when administered to nursing woman
Precautions & Warnings
- Adrenocortical insufficiency in patients with pituitary gland function decrease
- Careful dosing in patients with cardiovascular disease
- Dosage adjustment based on body weight change
- Monitoring blood levels of T3 and T4
Overdose Effects
- Signs and symptoms of hyperthyroidism
- Agitation, confusion, irritability, tachycardia, arrhythmias, coma
- Dose reduction or temporary discontinuation if signs of overdosage occur
