Indications
- Replacement therapy in hypothyroidism
- Suppression of Thyroid Stimulating Hormone (TSH) levels in goitres nodules and after treatment of Thyroid cancer
- Suppression of goitrogenic effects of other drugs
- Diagnostic aid in suppression tests
Pharmacology
- Contains synthetic Levothyroxine (T4)
- About 30% T4 is converted to the active Triiodothyronine (T3)
- T4 is protected by Thyroxine Binding Globulin
- The liver is the major site of degradation of Thyroid hormones
Dosage & Administration
- Adult dose: Initial starting dose 25-50 mcg/day
- Pediatric dosage varies based on age
- Dose adjustments based on clinical response and laboratory parameters
Interaction
- Concurrent use of tri/tetracyclic antidepressants may increase therapeutic and toxic effects
- Sertraline may result in increased Thyrolar requirements
- Addition of Thyrolar to antidiabetic or insulin therapy may increase requirements
Contraindications
- Untreated subclinical or overt Thyrotoxicosis
- Acute Myocardial Infarction
- Uncorrected Adrenal failure
Side Effects
- Adverse reactions primarily those of hyperthyroidism
- Includes fatigue, weight loss, nervousness, palpitations, diarrhea, hair loss
Pregnancy & Lactation
- Pregnancy Category A
- Caution advised during nursing, replacement doses generally needed
Precautions & Warnings
- Patients with hypothyroidism due to pituitary gland function may have adrenocortical insufficiency
- Care with dosing in patients with cardiovascular disease and severe hypothyroidism
- Adjustment of dosage required with marked change of body weight
Overdose Effects
- Signs and symptoms of overdose are those of hyperthyroidism
- Symptoms may not appear until several days after ingestion
- Dose reduction or temporary discontinuation if signs occur
