Indications
- Control of severe allergic conditions in asthma, atopic dermatitis, contact dermatitis, drug hypersensitivity reactions, perennial or seasonal allergic rhinitis, and serum sickness
- Treatment of collagen diseases like lupus erythematosus, rheumatoid arthritis, etc.
- Management of dermatologic diseases including bullous dermatitis herpetiformis, exfoliative erythroderma, mycosis fungoides, pemphigus, and severe erythema multiforme (Stevens-Johnson syndrome)
- Treatment of endocrine disorders such as adrenocortical insufficiency, congenital adrenal hyperplasia, hypercalcemia associated with cancer, and nonsuppurative thyroiditis
- Management of gastrointestinal diseases like regional enteritis and ulcerative colitis
- Management of various hematologic disorders like autoimmune hemolytic anemia, idiopathic thrombocytopenic purpura, and leukemias
- Management of nervous system conditions like multiple sclerosis, cerebral edema associated with brain tumor, craniotomy or head injury
- Treatment of ophthalmic diseases including temporal arteritis, uveitis, and ocular inflammatory conditions unresponsive to topical corticosteroids
- Induction of diuresis or remission of proteinuria in idiopathic nephrotic syndrome or due to lupus erythematosus
- Treatment of respiratory diseases such as pulmonary tuberculosis, eosinophilic pneumonias, and symptomatic sarcoidosis
- Adjunctive therapy for rheumatic disorders and miscellaneous conditions like trichinosis, tuberculous meningitis, etc.
Pharmacology
- Dexamethasone is a synthetic glucocorticoid
- Decreases inflammation by inhibiting the migration of leukocytes
- Reverses increased capillary permeability
- Suppresses normal immune response
Dosage & Administration
- Oral dosages vary from 1 mg to 10 mg for adults and 0.03-0.20 mg/kg body weight for children
- Higher dosages may be temporarily required in some patients
- Dosing depends on the severity of the condition and patient response
- Intravenous, intramuscular, or local injection administration
- Dosing for systemic emergencies like anaphylaxis and acute severe asthma
Interaction
- Enhanced potassium loss with diuretics and/or cardiac glycosides
- Potential impairment of glucose tolerance with antidiabetics
- Increased risk of gastrointestinal ulceration with non-steroidal anti-inflammatory drugs
- Alteration of need for oral anti-coagulants by glucocorticoids
Contraindications
- Gastric and duodenal ulcers
- Systemic and ophthalmic fungal infections
- Viral infections like varicella and herpes genitalis
- Glaucoma
- Hypersensitivity to corticosteroids
Side Effects
- Cushing-like syndrome
- Hirsutism
- Fluid and electrolyte disturbances
- Osteoporosis
- Gastro-intestinal effects like ulceration and haemorrhage
- Central Nervous System effects including euphoria and convulsions
- Immunosuppressive effects
Pregnancy & Lactation
- Category C for pregnancy
- Potential presence in breast milk
- Mothers taking high dosages advised not to breast-feed
Precautions & Warnings
- Use the lowest possible dose to control the condition
- Caution in patients with congestive heart failure, hypertension, or renal insufficiency
- Minimize drug-induced secondary adrenocortical insufficiency by gradual reduction of dosage
Use in Special Populations
- Titrate pediatric patients to the lowest effective dose to minimize potential growth effects
